Risk Factors
Epidemiologic factors (e.g. age, race, gender, smoking), risk-conferring diagnoses (e.g. w/ defined temporal relationship), family history
Associated conditions
Diagnoses that correlate, but have no defined temporal relationship (may share a pathophysiologic mechanism)
Pathophysiology
Etiologies with a known or theoretical mechanism. Often diseases that cause another disease.
Symptoms
To avoid repetition, symptoms can be summarized as syndromes or other diagnoses (e.g. write CHF instead of dyspnea, fatigue, cough, orthopnea, PND)
